Single Occupancy Assisted-Care
Autumn Leaves provides single-occupancy, assisted-care accommodations for individuals who can no longer meet their needs at home or who face ongoing and/or complex health concerns that require comprehensive support. We are committed to delivering compassionate care that respects each resident’s dignity, preferences, and personal choices. Our approach goes beyond medical care, with our interdisciplinary team working closely with aides, social services, family members, spiritual support, and most importantly, the residents themselves. From the moment of admission, we encourage residents to bring personal furniture and belongings (as space allows) to create a warm, homelike environment. Each private room is designed to offer comfort and familiarity, ensuring residents feel secure and at ease.
Dual Occupancy Assisted-Care
For couples or loved ones wishing to stay together, our dual-occupancy rooms provide a comfortable space to share, ensuring you don’t have to be separated from those who matter most.
Included Services
For Single & Dual Occupancy

Three meals and three snacks daily. Coffee, cookies, fruit, and beverages are available at all times.

Complete laundry services for linens, bedding, and personal belongings.

Assistance arranging transportation with families to medical appointments during regular business hours (8:00 a.m. - 4:00 p.m. Mon. through Fri.).

Group Activities.

Medication management, only by written orders from a physician.

Twenty-four-hour awake staff supervision.

Bed checks every two hours from 10:00 p.m. - 6:00 a.m. (Residents’ choice).

Assistance with showering twice a week.

Personal cell phones will be used for making calls, with staff help if needed.
Independent Living

Our facility offers four independent living apartments where all maintenance and outdoor upkeep are fully managed for you. Plus, you'll enjoy one catered meal each day, giving you the ideal balance of independence and support.
